What is it?
eSalut is a project of the Hospital Germans Trias i Pujol (Catalan Health Institute) in which, through a mobile application, patients are offered personalized information to help them monitor their illnesses from home.
The mobile application facilitates communication between the patient and the professional, while also involving the user in their healthcare process and improving their quality of life.
Who can access it?
Only those people who have been registered by the healthcare team within the application can use it. eSalut is available in both the Apple Store and the Google Play Store. The first to try this tool were users of the Hospital Germans Trias and some Primary Care Centers (CAP) of the Metropolitan Primary Care North in Badalona.
What can the patient find in the application?
The patient can find information, advice, guidelines and personalized exercises, etc., designed for each use case or pathology. The content has been prescribed, prepared and validated by the patient’s healthcare team. The tool helps to improve patient monitoring and marks the most appropriate control times in a personalized way, according to the patient’s clinical situation at all times.
The eSalut app currently has content for 60 different pathologies or processes, such as persistent Covid, heart surgery, colorectal cancer, pain, stroke, diabetic foot or home hospitalization, among others.
