- An RIS3CAT communities project and ERDF operational programme in Catalonia 2014-2020 to deploy a new model of integrated care for chronic patients.
01. Description
The project’s ambition is to deploy a new integrated care model for the adaptive management of complex chronic patients based on intelligent ICT solutions. The solutions will be based on 4P medicine (predictive, preventive, personalised and participatory). It will thus be able to manage personalised clinical pathways, offer the patient recommendations for self-management, and improve their quality of life by reducing the risk of disease relapse.
