The Government’s commitment to achieving greater integration of the social and health systems has taken an important step forward for the benefit of both professionals and users. A few weeks ago, the social and health department information systems began to be interconnected, so healthcare professionals now have direct access to view the dependency or disability data of the people they serve, while those in social services can view a minimal set of clinical history data for each person.
This will allow professionals in both areas to work more accurately and comprehensively, as they can see all the complexities of the people they care for, the support they receive from both systems, and the workers who care for them. It will also have an impact on people’s greater well-being, especially the elderly, people with disabilities, mental health problems and addictions, and generally anyone with complex care needs and those forming part of their care environment, as these individuals are often attended by both the social and healthcare systems.
There are already twenty core areas of social services and more than 498 professionals who have the viewer and Social + Health microviewer, where they can view information related to the health of their users, for example, their diagnoses, chronic condition markers, levels of frailty, whether they receive home care or what their healthcare team of reference is. The Social + Health microviewer is being rolled out and will reach the 109 basic social services in Catalonia and, in the future, access will also be possible from the different centres and services of the public social care network.
As for the Department of Social Rights, it has enabled the Social + Health microviewer with the social data it has, such as those on dependency or disability, among others, which are incorporated into the viewer of the Shared Clinical History of Catalonia (HC3) of the Department of Health. Therefore, all the health professionals and social workers of the Public Health Network will be able to consult this information through the viewer of the Clinical Health Record (HES). This access has been opened to all areas of the Public Network of Health.
The interconnection of the two information systems is the result of the Government’s commitment to comprehensive social and health care, which aims to provide the best possible care for the elderly, dependent, disabled or those with complex needs.
