The European project ASSEHS releases the Appraisal Standard dashboard

The European project ASSEHS (Activation of Stratification Strategies and Results of the interventions on frail patients of Healthcare Services), which started in 2014 and will end in June 2016, is an international initiative to bring together stratification related professionals from Health Services, Academia and Research Centres from European countries to study current existing health risk stratification strategies and tools and the challenges to extend their use and application on frail elderly patients.

Coordinated by Kronikgune (International Centre of Excellence in Chronicity Research), the project has had partners in different European regions: Montpellier, Southern Italy, the Basque Country and Catalonia. Fundació TicSalut has led section 6, regarding the Impact on structures and processes.

These are some of the different goals of the project:

• To define a map of Stratification of Risks in Europe.
• To obtain a map of tools.
• To define implementation and development strategies in different regions.
• To evaluate the impact on structures, processes and outcomes.

In the last part of the project, ASSEHS has issued the Appraisal Standard Dashboard, a comprehensive instrument that allows the System to be navigated. In this article, we talk about the website with the results.

In the next few days, the TicSalut website will make available a series of videos of the Online Course of Stratification of Risks, with different modules such as “Stratification of Risks in Catalonia and the GMA”, “Tools of Stratification of Risks” and “Usefulness of the Morbidity Groupers”.
